Pre-registration For your convenience, please print and complete the following forms prior to your arrival in our office.
BEFORE YOU ARRIVE: Be sure that all antihistamine use has been discontinued for at least one week. We will be unable to perform allergy skin testing if you have taken any antihistamines. Not all allergy related problems require skin testing, please call if you have any questions concerning this issue. If you are not sure if you are taking an antihistamine, consult your pharmacist.
