A key component to this system is the “Your Health file” patient portal. This is a secure cloud-based storage system that allows you to view your testing results after we have reviewed them. Every patient who receives care at our office is required to create an on-line medical record and provide us with accurate contact information. If you do not have computer access at home, we will allow you to use our laptop in our waiting room. To begin creating your medical record portal, please click our web site button “patient portal log-in” or go to: https://www.yourhealthfile.com/portal/login.jsp and sign in. This web site works best with the Google “Chrome” browser, if you are using “Internet Explorer”, you can easily download a Chrome browser at https://www.google.com/chrome/. When you call our office to schedule an appointment (210-499-0033), we will provide you with a temporary user name and password required to create your web portal. We ask that you go to your portal and complete your personal profile information and list of all your current medications, your past medical history, medication allergies, family history and social history. Once this has been done, you will have future access to this information from any computer or cell phone with internet access. You can simply print a copy for any physician who sees you in your future and you can immediately provide them with a copy of your labs, X-rays, skin test results and office visit discharge instructions. Most importantly, if you need emergency care, you can make this critical medical information available to any treating physician. In addition, once you complete your portal information, we can send prescribed medications electronically directly to your preferred pharmacy. *Always keep your health file up to date.
