When calling for an appointment, please be prepared to provide the patient's name, address, phone number, birth date, type of insurance, reason for appointment, and referring physician, if applicable.

Our office hours are as follows:

Open	8:00 a.m.	8:00 a.m.	7:30 a.m.	8:00 a.m.	8:00 a.m.
Close	5:00 p.m.	5:00 p.m.	12:00 p.m.	5:00 p.m.	5:00 p.m.

*We are closed for lunch daily from 12 noon to 1:30 p.m.
*Allergy shots will not be given after 11:30 a.m. or 4:30 p.m. (No exceptions)
